How to get your GP record online
Most patients will automatically be given access to information added to their GP record from November 2023 onwards. This includes letters, test results and appointment notes. Some people may also have access to information added to their GP record before this date. You need to be signed up to the NHS App or Online Services in order to access this information.
You can access your GP record by logging into your account using the NHS app or NHS website, or by registering for Online Services.
First, you need to register for online services and prove who you are. You can do this when you create an account.

If you do not already have access to Online Services please conact the surgery to register.
Subject Access Request
Patients can request access to their medical records via a Subject Access Request.
This service is free of charge.
Requests can be taken verbally or in writing.
Completed forms should be sent to the practice for processing.
