What is Social Prescribing?
Social Prescribing is suitable for any patient whose health and wellbeing is being impacted by practical or social issues. For example:
- Benefits and money
- Employment
- Housing
- Family
Our Social Prescribing Team adopt a personalised approach to each patient's circumstances and needs, understanding what matters to them and what their priorities are. From this, a plan is a developed to build their confidence in tackling in day to day issues, connect them to appropriate services and support and enable them to make informed choices moving forward. The team work collaboratively with local groups, voluntary sector organisations and statutory services, to strengthen accessibility and sustainability for the wider community.
